Understand the buyer before you apply
A transportation broker, managed-care plan, hospital, medical group or state agency may use a different provider application and insurance exhibit. Identify the legal contracting entity, state, counties, service modes and proposed start date. Distinguish an inquiry about joining a network from an awarded contract. Neither an insurance quote nor a certificate guarantees contracting, passenger referrals or reimbursement.
MTM Health and Modivcare requirements
Use the program-specific documents supplied for the work you intend to perform. MTM Health describes provider oversight that includes licensing, background checks, certifications and insurance. Legacy LogistiCare or Veyo references in older documents should be clarified with the buyer rather than counted as separate new contracts.
Other transportation networks to identify
Verida contracts with independent transportation providers, and SafeRide Health offers participation in its transportation network. Medical Answering Services is relevant to New York Medicaid transportation. These are additional names to recognize when organizing a submission, not endorsements or promises of local availability. New York has a specific enrollment checkpoint
For the non-medical transportation enrollment applications covered by eMedNY’s Option 2 instructions, a MAS support letter is required. This requirement is distinct from a carrier’s underwriting approval and illustrates why a generic national insurance checklist cannot replace state-specific enrollment review.
California plans and healthcare organizations
Medi-Cal managed-care work, direct medical-facility agreements and private-pay trips can coexist in one business. Explain how rides are authorized and who assigns the vehicle. Keep DHCS enrollment, any applicable operating permits and the insurance file consistent with the service provided. Hospitals, dialysis centers, rehabilitation programs and senior communities may request terms that differ from a transportation broker’s standard exhibit.
Other agency and paratransit agreements
A public-agency solicitation can include procurement, accessibility, service-performance and insurance provisions. Social-service trips and demand-response transportation may have passenger needs beyond medical appointments. Describe escorts, handoffs, complaint handling and subcontractors. Regional California, Arizona and Nevada operations should list each service territory and authority rather than relying on the fleet’s home-state paperwork for every trip.
Coverage lines to compare with the exhibit
Check commercial auto liability, physical damage where needed, general liability, professional liability, SAM, workers’ compensation and employer’s liability. Evaluate umbrella or excess coverage against the precise underlying schedule. Hired/non-owned auto, uninsured/underinsured motorists, medical payments, cyber, crime, business property and portable equipment may also be relevant. Any requested limit, including high excess capacity, remains subject to market availability and underwriting.
Vehicles, assistance and passenger populations
Provide separate details for wheelchair vans, gurney or stretcher vans, ambulatory vehicles and accessible shuttles. Record lift or ramp equipment, wheelchair positions, conversion values and attendant duties. Include dialysis, adult day care, senior transportation, behavioral health, hospital discharge and private-pay work. Explain emergency or clinical services separately so they are not inadvertently presented as routine non-emergency transport.
Driver credentialing and training records
Maintain the qualifications, background checks, driving records, inspection evidence and training documentation requested by the program. Record expiration dates and who monitors them. Passenger assistance, restraint-system use, lift maintenance and incident reporting deserve written procedures. A specific training brand, driver age or vehicle-age cap should only be described as mandatory when the applicable authority or buyer actually requires it.
Dispatch technology and trip opportunities
Describe dispatch platforms, trip tracking, subcontractor access and passenger-data handling. Software participation does not itself provide operating authority or insurance. Document who responds to missed pickups and unavailable vehicles. In a hypothetical Houston medical-campus contract, late discharge times and long return trips can affect scheduling, mileage and driver fatigue; a well-supported plan is more useful to underwriting than an unsupported claim of unlimited capacity.
Claims, premises and business continuity
Illustrative exposures include loading injuries, wheelchair securement allegations, parking damage at a hospital, an employee strain and damage to several vans at a shared depot. Review which policies may respond and how deductibles and exclusions operate. Include spare equipment, the dispatch office and any replacement-vehicle arrangement. These examples are planning tools, not verified claims or assurances that a settlement will be paid.
Certificates and endorsements are different documents
List the exact certificate holder and requested additional-insured entity, along with primary/noncontributory terms, waivers and cancellation language. What affects cost and what to send
Markets evaluate vehicle count and values, service level, territory, driver experience, claims, limits and deductibles. Provide declarations, available loss runs, unit and driver schedules, revenue, payroll, contracts and a list of proposed changes. New ventures can include relevant experience and a startup plan.
